Job Description
Provides physical therapy intervention and treatment.

1. Understands, keeps up-to-date, and provides services, intervention and treatment consistent with Department of Health (DOH) & CHAP requirements.

2. Evaluates patient for physical therapy in accordance with the agency policy and regulatory requirements.

3. Establishes and provides a plan of treatment for the patient based on results of the evaluation visit, client needs and physician orders.

4. Involves the patient and appropriate others in the planning, implementation and assessment of the intervention/treatment program.

5. Completes an Oasis re-certification assessment as needed obtains appropriate admission consents as per agency policy and regulatory requirements.

6. Submits all evaluation paperwork within 24 hours of visit.

7. Complies Oasis discharge assessment as appropriate.

8. Provides an initial verbal report to the Nurse Coordinator within 24 hours of the initial visit.

9. Instructs and counsels patients and families about the prescribed therapy and the importance and relationship to his/her medical status and well-being.

10. Provides written instructions and educational materials to the patient as indicated.

11. Communicates with the Nurse Coordinator no less than every two weeks to report patient status, response to treatment, progress with goals, changes.

12. In consultation with appropriate disciplines, plans for discharge of the patient taking into consideration goal achievement, and provides for appropriate follow-up or referral.

13. Documents on an ongoing basis, services provided, and responses to services/treatment and changes in status relative to the plan of care.

14. Completes a note for each visit and submits documentation within 7 days of the visit.

15. Completes summaries of care every 60 days and at time of discharge.

16. Assumes responsibility for maintenance of condition of employment and continued professional growth

17. Assists with the supervision of staff, including orientation of new staff and participating in Peer review evaluations.

18. Ensures that services are provided in accordance with established policies and procedures and regulatory requirements.

19. Performs other duties as required or assigned.

Knowledge, Skills And Requirements
Bachelor’s degree (B.A.) or higher from a New York State Registered or American Physical Therapy Association (APTA) Accredited Physical Therapy Program and current license/registration to practice as a Registered Physical Therapist in New York State.

PT’s must be able to travel to make home visits in the Bronx, Brooklyn, Manhattan and Queens. OASIS experience required.
