Our Case Management for seniors program in Manhattan is looking for a Community Worker.

Job description:
The Community worker will provide comprehensive case management services for homebound seniors in Manhattan Community Districts 4 and 7 such as:
- Assessment
- Supportive counseling
- Referrals for services i.e.: meals on wheels, access a ride, etc.
- Advocacy
- Screening for entitlements and benefits
- Applying for entitlements and benefits
- Maintaining records and creating statistical reports
- Conducting home visits
Knowledge, Skills And Requirements
Bachelor's Degree Required
Geriatric experience preferred
Bilingual English/Spanish preferred.
Strong computer Skills (MS Word, MS Excel)
Organized
Good time management skills
