Job Description
Job Summary: Provides physical therapy intervention and treatment.

Job Responsibilities

1. Understands, keeps up-to-date, and provides services, intervention and treatment consistent with Department of Health (DOH) & CHAP requirements.

2. Evaluates patients for occupational therapy in accordance with Agency admission policy and regulatory requirements.

3. Establishes and provides a plan of treatment for the patient based on results of the evaluation visit, client needs and physician orders.

4. Involves the patient and appropriate others in the planning, implementation and assessment of the intervention/treatment program.

5. Submits all evaluation paperwork within 24 hours of visit.

6. Provides an initial verbal report to the Nurse Coordinator within 24 hours of the initial visit.

7. Instructs and counsels patients and families about the prescribed therapy and the importance and relationship to his/her medical status and well-being.

8. Provides written instructions and educational materials to the patient as indicated.

9. Communicates with the Nurse Coordinator no less than every two weeks to report patient status, response to treatment, progress with goals, changes.

10. In consultation with appropriate disciplines, plans for discharge of the patient taking into consideration goal achievement, and provides for appropriate follow-up or referral.

11. Documents on an ongoing basis, services provided, and responses to services/treatment and changes in status relative to the plan of care.

12. Completes a note for each visit and submits documentation within 7 days of the visit.

13. Completes summaries of care every 60 days and at time of discharge.

14. Assumes responsibility for maintenance of condition of employment and continued professional growth

15. Assists with the supervision of staff, including orientation of new staff and participating in Peer review evaluations.

16. Ensures that services are provided in accordance with established policies and procedures and regulatory requirements.

17. Performs other duties as needed or requested.

Knowledge, Skills And Requirements
Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience: Bachelor’s Degree (B.A.) from a four year college or university; or one to two years related experience and/or training or equivalent combination of education and experience.
Certificates, Licenses, Registrations: Current license to practice as a Registered Professional Nurse in New York State

Language Skills:
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manual. Ability to effectively present information and respond to questions from groups of manager, clients, and the general public.

Mathematical Skills:
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basis algebra and geometry.

Reasoning Ability:
Ability to solve practical problems and deal with a variety of concrete variables in situations where
only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Supervisory Responsibilities: No direct reports.

Physical Demands: Supervises staff in the administration of nursing procedures in the homes of clients.
Travels to the patient’s home, climb stairs to gain access.

Work Environment: Office and client's home.

OTHER SKILLS AND ABILITIES: Excellent communication, writing and organized skills. Sensitivity to vendor, client and staff needs and problem resolution skills.
